How to use this template
Copy the sections below into a document or print this page and keep it on a clipboard at the post. Fill in the header before the first round; make every entry as it happens, never afterward; and close the log when the system is restored and tested or the post-work period has run. Price Protection officers keep a log in this form on every watch and hand the original to the coordinator.

Filling it in
Write the header with the coordinator before the first round, and write the authority’s conditions exactly as given: the interval, the areas and how the alarm will be raised with the system down. In the rounds table, “areas walked” means every floor or zone on the list, named; “exits and equipment” means the doors, stairs, extinguishers and standpipe connections checked; “conditions found” is either “no hazards” or the specific thing seen. Do not leave a round out because nothing happened; the empty rows are what an inspector notices first.
Events are logged in the events table so the rounds table stays clean. A hazard is described, along with what was done about it and who was told. A change of officer is a handover entry with both names, and the incoming officer reads the log before signing.
The close is signed by two people because the log is evidence for both: the officer that the watch was kept, the coordinator that the building received it. Keep the original in the impairment file; keep a copy with the security company. The log inspection article explains what inspectors read for, and the sprinkler impairment checklist covers everything around the log.
Questions
Using this template.
Does the fire marshal require a particular log form?
Some jurisdictions and some inspectors do. Ask the fire code official when the watch is approved. This template covers what inspectors generally look for; use the authority's form if one is specified.
Can the log be electronic?
Usually, as long as entries are time-stamped as they are made, cannot be altered afterward and can be produced on request. Keep a printed copy in the building's impairment file at the end of the watch.
Who signs the log?
The officer making each entry initials it, the outgoing and incoming officers sign each handover, and the officer and the impairment coordinator or permit holder sign the close.
How long should the log be kept?
With the building's impairment records and the insurer's file. Inspectors, adjusters and lawyers may ask for it long after the system is restored.
